To become qualifed, you must meet the required criteria, and provide a signed copy of a "Suitability Questionaire", as required by various State and Federal Regulations. When specific projects are officially made avalilable to the public for investment opportuinities, "Suitability Questionaires" will be available. You can download those right here on our website. You'll find clearly marked links on the "Projects" page and in other sections when a new project is launched.

Signed copies of "Suitability Questionaires" must be kept on file for ALL qualified Investors. You can conveniently download them in PDF format and print them yourself, for each individual new project. (New Project to Be Announced) Fill out, sign and mail them to:

To discuss any project immediatly with one of our customer service representatives, you may FAX your "Suitability Questionaire" to our offices at FAX: 1.972.788.2563.

Please Note: Even if you FAX your Questionaire, YOU MUST STILL MAIL a paper copy of your signed Questionaire to our offices in order to receive your access code. We must have a signed copy of your "Suitability Questionaire" in our files. NO PERSON may invest under any circumstances without submitting their signed "Suitability Questionaire".

Once you are qualified, (after mailing or FAXING first, then mailing your "Suitability Questionaire") we'll email you your personal "login code". Accredited investors can login in the box located on the "Pre-Qualified" page. Select the "I'm Pre-Qualified" button on the menu to access the login.

The Suitability Questionaire is a confidential document, and will be dealt with as such by Oil America Group.
